As a new patient at the Center for Neurosciences you will be required to complete several forms. You will receive these forms by mail in your new patient welcome packet, but you may also download them here.

Referrals
It is our policy that a physician referral is required in order to schedule an appointment.
You will be able to make an appointment for future visits at check-out. To best ensure your desired appointment time, please make follow up appointments prior to leaving our offices. We will do our best to make an appointment that is convenient for your schedule, but we cannot guarantee the appointment date and time you request will be available.
Your copayment or coinsurance, in accordance with your health insurance plan, will be due at the time of your appointment. We accept cash, personal checks with proper identification, Visa, MasterCard and American Express.
If you need to change or reschedule an appointment, please contact our office at (520) 795-7750 no less than two (2) days in advance of your scheduled appointment.
